This assessment is intended for community organizations, health leaders, nonprofit partners, government representatives, and other local stakeholders requesting support or exploring collaboration with Montero Medical Missions.

Before you begin
- Allow approximately 15–25 minutes to complete the assessment.
- Have basic community statistics, partner information, and facility details available.
- You may be asked for supporting documents or non-identifiable photographs.
- Do not include patient names, medical records, or protected health information.
- Submission does not guarantee selection or funding; every request is reviewed for alignment, feasibility, partnership, and sustainability.

What happens after submission?
You will receive a confirmation and reference number. Our team will screen the request, follow up if information is missing, and route it to the appropriate program reviewers. Complex opportunities may require partner verification and a separate feasibility discussion.
